If you prefer to make your purchase using a personal check, you may do so using the 2CheckOut.com web site or also by using an order form available in either Microsoft Word or Adobe PDF format.

After printing the form, click on your browser "Back" button, if available, to return to this page. If a "Back" button is not available, close the "Order Form" page to return to this page. Mail your completed form to the address below.
